Purpose

This award was established in 2017 to recognize a person, group of people, or entity from outside the PT profession for substantial contributions that have propelled our chapter toward the realization of our vision and strategic plan.

Eligibility

• Nominee must be an a non-physical therapist or non-physical therapist assistant
  
  

Criteria

The recipient will have contributed professional skills and knowledge that have positively impacted APTA MN and/or the profession of physical therapy. Demonstrated by:

• Volunteerism of professional skills and/or knowledge applied to the APTA MN vision and/or strategic plan.

• Demonstration of professional stature and ethics.

• The effect their contributions have had on the growth of the society or profession.

• Initiative, imagination, dedication, and other professional attributes.

Method of Nomination

• Nominations must be recommended for the award by APTA MN members in good standing.

• Letters of reference should be directed to APTA MN Awards Committee at info@mnapta.org

• Nominations must be made on an APTA MN Nomination Form. Please provide documentation of the candidate’s specific activities and contributions which qualify him/her for this award, according to the outline of the criteria.

• Nomination deadline is February 15, 2024.

Selection of Recipient

• A task force will be appointed annually by the Board of Directors. This task force will have the responsibility of evaluating the nominees and selecting the recipient.

• Awards will be made only if contributors are of truly outstanding character. Awards are not necessary each year.

• The award will be presented at the Annual Conference.
