Corinne Ellingham Outstanding Service Award Nomination

Purpose

This award has been established to honor an APTA Minnesota member who has made an outstanding contribution to physical therapy.


Eligibility

Any active member in good standing of APTA MN.


Criteria

The recipient will have demonstrated consistent service to the development of the physical therapy profession beyond that inherent in being a member of the profession. This commitment of services is demonstrated by a contribution to the profession in the following areas:

The Profession of Physical Therapy (PT Only) Education- The recipient has fostered the development of quality learning experiences that promote sustaining and improving the physical therapy profession. These include:

  • Teaching in any of the following areas: academic, clinical, in-service, or continuing education
  • Development and implementation of teaching methods
  • Administration of physical therapy program

(PT Only) Clinical Research- The recipient has fostered laboratory and/or clinical research that has promoted the development of the physical therapy profession and has published the disseminated results of his/her research. Service- The recipient has demonstrated service of the profession in patient care, consultation, administration or education and as a liaison to other health care professions, organizations and/or communities. For example, community Task Force for Handicapped Housing, The Arthritis Foundation, P.S.R.O.

 

The American Physical Thearpy Association or Its Components

  • Promotion of the organization or its goals
  • APTA MN or national elected offices
  • APTA MN or national committee or task force memberships
  • Community Service Organizations
  • Local, state or federal government on elected or voluntary basis


Method of Nomination

  • Nominations for the award must be submitted by a member of APTA MN in good standing.
  • Letters of reference should be directed to the Awards Committee at info@mnapta.ort.
  • Nomination deadline is February 15, 2024.
  • Nominations shall be submitted in writing using the appropriate form. In addition, provide documentation of the candidate’s specific activities and contributions which qualify him/her for this award, according to the outline of the criteria. Please consider the following areas:
  • Active participation in APTA MN or APTA Demonstrated professional stature and ethics
  • The effect their contributions have had on the growth of the profession
  • Initiative, imagination, dedication and other professional attributes


Selection of Recipient

  • A task force will be appointed annually. This task force will have the responsibility of evaluating the nominees and selecting the recipient.
  • Awards will be made only if the contributions are truly outstanding in character. No more than one award will be given each year. Awards are not necessary each year.
  • The award will be presented at the Annual Conference.
